1. ACKNOWLEDGMENT OF RISK AND LIABILITY WAIVER
I, the undersigned parent or legal guardian of the participant named above, hereby acknowledge and agree to the following:
1. I understand that participation in Football Lab training sessions, including but not limited to soccer drills, scrimmages, and other physical activities, involves inherent risks, including the risk of injury, illness, or death.
2. I voluntarily assume all risks associated with my child's participation in the training sessions.
3. I agree to release, discharge, indemnify, and hold harmless Football Lab, its directors, officers, employees, volunteers, and agents from any and all claims, damages, or liabilities resulting from my child's participation in the training sessions, except in cases of gross negligence or intentional misconduct.
2. MEDICAL AUTHORIZATION
In the event of an emergency where I cannot be reached, I authorize Football Lab and its representatives to arrange for medical treatment, including transportation to a medical facility, for my child. I understand that I am responsible for all costs related to medical treatment.
3. MEDIA RELEASE
I grant permission to Football Lab, as well as its trusted contractors and affiliates engaged on a case-by-case basis, to capture and use photographs, videos, or other media taken during training sessions. These may be used for Football Lab's promotional purposes, including but not limited to social media, websites, and printed materials. I understand that no personal identifying information (such as my name or address) will be shared without my consent.
4. CODE OF CONDUCT
I agree to ensure my child adheres to the Football Lab Code of Conduct, which promotes sportsmanship, respect for others, and adherence to safety protocols during all training sessions. I understand that failure to comply may result in removal from the program without refund.
5. GENERAL AGREEMENT
By signing this form, I confirm that:
• I have read and understood this waiver and release of liability.
• I am the legal parent/guardian of the participant and have the authority to sign this document.
• This waiver is binding for all Football Lab training sessions in which my child participates, unless revoked in writing.
